PROJECT OVERVIEW

NSF/NIH-funded VIP research project.
Patient mobile app and clinician web portal.

A connected mobile app and clinician portal that helps older adults and people with cognitive impairments set fall-prevention goals, track them at home, and stay supported by their care team.

Goal is to redesign and improve Version 1

PROJECT CLIENT

NSF/NIH-funded Vertically Integrated Project research project.

PROJECT ROLE

PROBLEM STATEMENT & ITS AUDIENCE

Fall prevention doesn’t end at the clinic door.

Clinicians can design a personal plan. Following it daily, without help, is the hard part, and clinicians can’t see what happens between visits.

How might we bridge clinician-guided planning and independent action at home?

SOLUTION

From Fall-Prevention Goals to Everyday Action.

SteadyPath bridges the gap between clinical guidance and everyday life. Designed for older adults, individuals with cognitive impairments, and healthcare clinicians, the platform transforms personalized fall-prevention goals into manageable daily activities.

Through guided routines, reminders, and progress tracking, individuals can build safer habits at home, while clinicians stay connected, monitor engagement, and provide timely support.

CONNECTED CARE & PROGRESS TRACKING

RESEARCH AND EVALUATION

Understanding the current app first.

Three phases informed the redesign without making usability testing the centerpiece.

Targeted Users

Two roles, four connected experiences.

SteadyPath helps patients set and track fall-prevention goals, and helps clinicians guide, monitor, and follow up remotely.

INFORMATION ARCHITECTURE

Structuring Two Connected Experiences

Two navigation models built around different jobs, joined by shared goals, activity records, and communication. Hover a shared link to trace it.

USER FLOWS

Designing the End-to-End Care Journey

Four flows hand off across roles: the clinician’s goal becomes the patient’s task, and the patient’s check-in becomes the clinician’s signal.

USER FLOWS

Designing for Cognitive and Physical Accessibility

Built for older adults and people with cognitive or physical impairments, including traumatic brain injury. Each principle maps to a SteadyPath decision. WCAG 2.2 AA and W3C mobile guidance are design targets

DESIGN PROCESS

From fragmented workflows to a structured interface

Eight steps, from evaluating the old app to refining accessible screens.

WIREFRAMES

Low to Mid-Fidelity - for User testing

Reconstructed conceptual wireframes: alternative layouts and information groupings for Patient Mobile and Clinician Desktop, explored before key decisions were made.

USABILITY TESTING AND FEEDBACK

Usability Testing & Feedback- Clinicians + Patients

Illustrative themes for the conceptual prototype, not verified research data.
These findings led to the accessibility-focused refinements in the final design.

01.

MOBILE-FIRST EXPERIENCE · INFORMED BY USER FEEDBACK

Patient Experience

The mobile experience supports older adults and individuals with cognitive impairments in following personalized fall-prevention goals, independently tracking daily activities, and staying connected with their care team. Designed for everyday use, it helps translate clinical guidance into manageable routines at home.

SteadyPath also adapts to larger screens, offering end users the same core features and flexibility to access their fall-prevention goals, track progress, and connect with their care team across devices.

DESKTOP & TABLET · RESPONSIVE EXPERIENCE

02.

DESKTOP-FIRST EXPERIENCE · INFORMED BY CLINICIAN WORKFLOWS.

Clinician Experience

A centralized workspace for proactive patient care.

The desktop portal brings patient activity, goal management, follow-up priorities, and communication into one workspace, supporting clinicians as they manage multiple patients throughout their workday.

MOBILE · RESPONSIVE EXPERIENCE

SteadyPath also adapts to mobile, allowing clinicians to access user information, monitor progress, review follow-ups, and coordinate care while away from their desktop workspace.

DESIGN SYSTEM

One Unified Design System, Built Around Accessibility and Scalability

A custom, accessibility-first, cross-platform design system informed by Apple’s Human Interface Guidelines, responsive web design principles, and WCAG 2.2 Level AA targets.
